- Right to a Paper Copy of This Notice – You may request a printed version at any time.

If you believe your privacy rights have been violated, you can file a complaint with our office or with the U.S. Department of Health and Human Services (HHS) Office for Civil Rights. We will not retaliate against you for filing a complaint.

We reserve the right to change this Notice at any time. Any changes will apply to PHI we already hold and will be available upon request and on our website.
