Mentally Free Psychiatry LLC is required by law to maintain the privacy of your Protected Health Information (PHI) and provide you with this Notice of Privacy Practices. We are committed to protecting your medical information and ensuring confidentiality as required by the Health Insurance Portability and Accountability Act (HIPAA).

How We May Use and Disclose Your Information

We may use and disclose your PHI for the following purposes without additional authorization:

  2. Payment – We may use and disclose your PHI to process payments for services rendered.
  3. Healthcare Operations – We may use your information for practice operations such as quality assessments and staff training.

  • Right to Amend – You may request corrections to inaccurate information in your records.
  • Right to Request Restrictions – You can ask us to limit how we use or share your PHI.
  • Right to Confidential Communications – You can request how and where we communicate with you.
  • Right to an Accounting of Disclosures – You may request a list of instances where we disclosed your PHI outside of treatment, payment, or operations.
  • Right to a Paper Copy of This Notice – You may request a printed version at any time.

If you believe your privacy rights have been violated, you can file a complaint with our office or with the U.S. Department of Health and Human Services (HHS) Office for Civil Rights. We will not retaliate against you for filing a complaint.

We reserve the right to change this Notice at any time. Any changes will apply to PHI we already hold and will be available upon request and on our website.
